Health and Wellness

Blue Cross Medical Insurance

Employees can choose between a PPO and HMO plan. Employees and their qualified dependents become eligible on their date of hire. Participants make a pre-tax paycheck contribution towards insurance costs, according to family status.

Delta Dental Insurance

Delta Dental Premier provides comprehensive dental benefits starting on date of hire.

Domestic partners and their children are eligible for coverage under both the medical and dental insurance plans.

 

Work/Life Balance

Vacation

Vacation accrues at an annual rate of 15 days per year in the first complete year of service.

Holidays

Phase Forward offers 12 paid holidays each year, consisting of a combination of fixed and floating holidays.

Family and Medical Leave

Employees are eligible for leave time under the Family and Medical Leave Act (FMLA) once they have worked for the company for at least 12 months (and a minimum of 1,250 hours). FMLA provides twelve weeks of job-protected, unpaid leave for childbirth, for placement of a child for adoption or foster care, for care for an immediate family member with a serious health condition, or for an employee's own personal serious health condition.

Parental Leave

After six months of service, employees are eligible for five paid days for the birth or adoption of a child.

Adoption Assistance Program

After three months of service, employees are eligible for up to $4,000 per adoption for assistance with adoption-related expenses.

Employee Assistance Program

Phase Forward offers the LifeScope EAP program through Sobel & Raciti Associates. This program includes free and confidential counseling services, assistance finding childcare, and referrals for legal and financial help.

 

Financial and Income Protection

401(k)

Phase Forward offers both a 401(k) and Roth 401(k) plan. Employees become eligible on the first of the month following their hire date regardless of the number of hours they are scheduled to work.

Short Term Disability

After an elimination period of 7 calendar days, this insurance provides 100% of regular base salary for the first four weeks and 60% thereafter for up to the 90th day of disability. There is no cost to employees for this plan.

Long Term Disability

Prudential Long Term Disability insurance provides 60% of base salary (up to a $10,000 per month maximum), in the event that an employee is disabled for more than 90 days. Phase Forward pays the premiums on the LTD insurance and imputes the amount of this premium on the W-2 so that employees pay tax on the premium amount. LTD payments are not taxed.

Life Insurance and Accidental Death and Dismemberment

Phase Forward will provide two times your annual salary in term life insurance as well as a Business Travel Accident plan.

Flexible Spending Accounts

Phase Forward offers a full Section 125 Plan. Employees may withhold (pre-tax) up to $3,000 for out-of-pocket medical/dental expenses as well as $5,000 for dependent care. Both the Healthcare and Dependent care spending accounts have a debit card option to ease the utilization of this benefit.

 

Education

Tuition Assistance

Phase Forward will reimburse employees up to $5,000 per year for job-related undergraduate and graduate level courses (completed). The level of reimbursement is dependent upon the grade received.

529 CollegeBound Fund

Employees can have money deposited directly into an account for their own or a family member's college or graduate education.
